We would like to audio and/or video tape your presentation at Pacific Symposium 2007. Audio tapes may be available for immediate distribution to attendees. Your colleagues will be able to benefit from your remarks by listening to the tapes whether or not they were in attendance. The audio tapes will be copyrighted and marketed under Title 17 of the U.S Code or other law that may be enacted. All rights to royalties, if any, in conjunction with tape sales shall hereby be assigned to the Pacific Symposium. This agreement does not preclude the publications, by you or your publisher, of your paper, speech, or comments at any time and in any format. Please cooperate with out audio/video staff to obtain the best possible recordings by using the microphone at all times and repeating all questions asked by persons without a microphone.

AUDIO RECORDING

I hereby give permission to Pacific Symposium and/or Pacific College of Oriental Medicine to record and distribute CDs containing my presentation as outlined above.
